    JOB DESCRIPTION

    TITLE: Rural Care Coordinator - Floater
    REPORTS TO: Member Services Manager -- Rural Region
    SUPERVISES: No one
    BASED AT: Access to Healthcare Regional Office(s) as Assigned
    Status: Non-Exempt

    JOB PURPOSE: The Regional Rural Care Coordinator provides a diverse set of Access to Healthcare services for residents of a specific multi-county catchment area. This employee covers a large geographic area and may utilize multiple AHN office location(s) on a rotating basis. The role of the Regional Rural Care Coordinator is to facilitate new applicants through the system from initial enrollment and orientation to eventual case management services when needed. This employee ensures that AHN members meet their obligations to both AHN and the provider network. A care coordinator will serve as a patient advocate, trouble-shoot issues and help patients navigate what can be a complex healthcare system while guiding them with a treatment plan laid out by their providers with firm costs for their co-pay. The Rural CC conducts provider and Employer relation functions as well.

    KEY RESPONSIBILITIES AND ACCOUNTABILITIES:

    Provide Regional Rural Care Coordinator services noted below, at assigned location(s), which will include travel to multiple sites in various communities.
    Field inquiry calls from interested parties, including potential members and employers. Explain program requirements, make timely disposition of calls and take appropriate actions on inquiry calls and referrals. Return all potential member calls screened by the AHN call center
    According to HIPAA requirements, confidently and discreetly obtain information from potential AHN members for eligibility/enrollment purposes
    Effectively and accurately enroll new members into AHN
    Effectively and confidently provide program orientation for new AHN members
    Effectively case manage their assigned AHN members to include management of regional provider referrals, generating price quotes, researching/locating available resources for member utilizing the AHN Rural Member Services Manager for assistance as needed
    Develop and maintain effective working relationships with regional AHN providers and members
    Understand the key role our providers play in the program and conduct oneself accordingly
    Accurately and in a timely manner enter member and provider information into the AHN software system
    Conduct provider In-Services ensuring providers understand AHN process and systems
    Manage provider network troubleshoot and solve provider issues and maintain provider network that enjoys our program
    Help to educate AHN members on the workings of the health care system as needed
    Refer AHN members to appropriate regional social service programs
    Perform general administrative duties for Access to Healthcare Network when needed and/or requested


    OTHER REQUIREMENTS:

    Must be willing to drive throughout Rural Northern Nevada on a regular basis to enroll, orient and care coordinate AHN members in the different regional offices
    Ability to be both compassionate and professional while serving our population
    Gain clear and working knowledge of Access to Healthcare Network services available to our members
    Gain clear and working knowledge of the provider referral and payment process
    Maintain and update the Regional Care Coordinator Manual for the rural area assigned
    Gain a clear and working knowledge of the Access Health Software system used in tracking demographics and service data, case management and member referrals
    Gain a clear and working knowledge of the Nevada Check-Up, Medicare and Medicaid enrollment criteria and process as well as terminology and practices
    Gain clear and working knowledge of social services available throughout our community; establish and maintain appropriate working relationship with persons and agencies involved
    Gain a clear and working knowledge of all Access to Healthcare Network administrative duties
    Able to sit and work at a computer for up to four hours at a time
    Be able to lift up to 30 pounds with a fair consistency
    Must attend periodic staff meetings and be willing to meet with the Rural Manager on a consistent and regular basis, and
    Must attend periodic training in Reno, NV and other locations. 15% of job is estimated to consist of regional travel requirement with mandatory periodic overnight travel required to accommodate initial or on-going training needs.

    QUALIFICATIONS
    Two years working in health care related social work, with experience in medical billing and patient coordination required.
    Bachelor's degree in social work, public/community health administration, sociology, psychology or related field preferred or equivalent education/experience.
    Minimum one year computer experience including Microsoft operating systems, word processing, spreadsheets, data base input and report generation.
    Excellent customer service skills
    Spanish bi-lingual preferred
    Must possess excellent communication skills both written and oral
    Must have reliable transportation and possess a valid Nevada driver's license
